Wisdom teeth usually show up between ages 17 and 25, and for many people there just isn’t room for them. When they come in crooked, get stuck under the gum, or crowd the teeth in front, removing them can prevent pain and bigger problems. At Siena Dental, we remove wisdom teeth in our Leander office, with sedation available so you can relax.
Do Wisdom Teeth Always Need to Come Out?
No. If your wisdom teeth are healthy, fully in, and easy to clean, you may be able to keep them. We’ll check them with X-rays at your regular visits and only recommend removal when there’s a good reason.
Signs Your Wisdom Teeth May Be a Problem
- Pain or pressure at the back of your mouth
- Swollen, tender, or bleeding gums behind your molars
- Jaw stiffness or trouble opening your mouth
- Bad breath or a bad taste that won’t go away
- Food getting trapped around a partly erupted tooth
- Crowding or shifting of your other teeth
Impacted Wisdom Teeth
A wisdom tooth is impacted when it can’t fully break through the gum. It may be trapped under the gum or bone, or growing in at an angle toward the tooth in front of it. Impacted teeth can lead to infection, damage to neighboring teeth, and cysts, so we often recommend removing them before problems start.
What to Expect
Consultation
We take X-rays, and a 3D CBCT scan when needed, to see the exact position of each tooth and its roots in relation to nearby nerves and sinuses.
Comfort
The area is fully numbed. You can also choose sedation: nitrous oxide, oral sedation, or advanced sedation with a local anesthesiologist. Many patients choose sedation for wisdom teeth.
Removal
Depending on the tooth, your dentist may make a small opening in the gum and remove the tooth whole or in sections. We close the area with stitches if needed.
Recovery
- Expect some swelling and soreness for two to three days. Ice packs help.
- Eat soft foods like yogurt, smoothies (without a straw), and soup for the first few days.
- Don’t use straws, smoke, or spit forcefully for at least 72 hours to protect the healing sockets.
- Take pain medicine as directed and rest for a day or two.
- Rinse gently with warm salt water starting the day after surgery.
Most people are back to school or work in two to four days.

What's the best age to remove wisdom teeth?
Often in the late teens or early twenties, when the roots aren't fully formed and healing tends to be faster. Adults can still have them removed safely. -

With advanced sedation from our partner anesthesiologist, you'll be in a deep, relaxed state and likely won't remember the procedure. With nitrous or oral sedation, you'll be awake but calm. -
Do all four have to come out at once?
Not always, but removing them in one visit means one recovery. We'll recommend what makes sense for you. -
What if my wisdom tooth hurts right now?
