We always try to save a natural tooth first. But when a tooth is too damaged, infected, or crowded to keep, removing it can end the pain and protect the teeth around it. At Siena Dental, Dr. Bashir Motamedi and Dr. Bruce Beheshtin handle simple and surgical extractions in our Leander office, with sedation available if you’re nervous.
When a Tooth Needs to Come Out
- Decay or a crack that goes too deep for a filling, crown, or root canal
- An infection that hasn’t responded to other treatment
- Advanced gum disease that has loosened the tooth
- Wisdom teeth that are impacted or crowding other teeth
- Extra or crowded teeth before orthodontic treatment
- A tooth broken in an accident or injury
Simple vs. Surgical Extractions
Simple Extraction
For a tooth that’s fully visible above the gum. Your dentist numbs the area, loosens the tooth, and removes it. You’ll feel pressure, but not pain.
Surgical Extraction
For a tooth that’s broken at the gumline, has curved roots, or hasn’t fully come in. Your dentist makes a small opening in the gum and may remove the tooth in sections. A 3D CBCT scan helps us plan around nearby nerves and sinuses when needed.
Staying Comfortable
Every extraction starts with local anesthetic. If you’d like more help relaxing, we offer sedation, from nitrous oxide to oral sedation, and we work with a local anesthesiologist for advanced sedation.
Planning Ahead for a Replacement
If you’ll want to replace the tooth, we can place a bone graft in the socket right after the extraction to keep the bone ready for an implant. In some cases, the implant can even go in the same day. Ask us about your options before your extraction.
Recovery Tips
- Bite gently on gauze for the first 30 to 45 minutes to control bleeding.
- Rest for the day and keep your head raised.
- Use an ice pack on your cheek, 15 minutes on and 15 off.
- Stick to soft, cool foods for a few days.
- Don’t use straws, smoke, or spit forcefully for at least 72 hours. This protects the blood clot and helps prevent dry socket.
- After 24 hours, rinse gently with warm salt water.
Call us if you have bleeding that won’t stop, pain that gets worse after the third day, or a fever.

Tooth Extraction FAQs
-
Does getting a tooth pulled hurt?
You'll feel pressure but shouldn't feel pain. Afterward, most soreness is manageable with over-the-counter pain relievers and eases within a few days. -
How long does recovery take?
Most people feel back to normal in three to five days. The gum closes over in a couple of weeks, and the bone fills in over a few months. -
What is dry socket?
It happens when the blood clot in the socket comes loose too early, exposing the bone. It's painful but treatable. Following your aftercare, especially avoiding straws and smoking, is the best prevention. -
Should I replace the tooth?
-
Can you see me the same day?
If you're in pain, call us. We hold time for same-day emergency visits Monday through Thursday.
